Apply for a Licence
If you are interested in applying for a licence or if you would like more information about the different types of licences, please follow the links below to download an application form and to view a sample licence. Follow the instructions in the applications regarding what information to provide and how many copies of the information to provide. Also follow the instructions in the application regarding payment of the application fee (which must be sent in at the same time as the application).
